Why bother if I compete with a medical form on file?

The medical form is with the organisers, not on the course. When you're treated at kilometre 30, nobody walks back to the tent for a sheet of paper: the QR puts that data where the emergency happens.

Does it survive the sweat of a long race?

It depends on the medium. Silicone wristbands and laminated stickers on the bib or watch perform best. The code itself doesn't degrade with use.

Is it useful for team sports or only endurance?

In contact sports it matters just as much: a concussion leaves a player unable to answer basic questions. Coaching staff scan it and know about prior conditions, medication or allergies.

Can I include my cardiologist or sports physician?

Yes. Beyond emergency contacts you can add the professional who follows your case, usually the most useful person for a doctor treating you without knowing your history.
